Care Management

McKenney Home Care has been providing Care Management services in Collier, Lee, Sarasota and Charlotte Counties since 2013. We have extensive experience with family members who are frustrated and frightened as a result of their inability to manage a loved one’s care due to time restraints, physical distance, family conflicts or lack of local or relevant knowledge.

McKenney maintains the highest level of professionalism, integrity and ethics. We provide comprehensive services which allow our Care Managers to maintain continuous care within one organization.

WHAT WE DO

Healthcare Management

Interact with the health care system, attend doctor appointments, manage medication and facilitate communication between doctor, client, and family. We help determine types of services – such as home health and hospice – that are right for a client and assist in engaging and monitoring those services.

Crisis Management

We assist promptly in the event of an unexpected health event. This includes a home or hospital presence as well as advocacy in the ER, hospital, rehabilitation facility. For families that live at a distance, this can be a much-needed 24/7 emergency contact.

Housing Maintenance and Transitions

We assist clients in maintaining their homes, including arranging for repairs and safety modifications. We also assist families evaluate and select appropriate housing or residential options.

Families

We work with families to adjust, cope and problem-solve around long-distance and in-home caregiving. We address care concerns, internal conflicts and differences of opinion about long-term care planning.

Local Resources

McKenney Care Managers know the local resources in the community and know how services are accessed.

Financial and Legal

We oversee bill pay and organize basic finances. We review long term care policies and work together with financial managers. We also refer to legal experts, like elder law attorneys, estate planners, and Powers of Attorney.

Advocacy

We are advocates for clients and their families, promoting the client’s wishes with health care and other providers, ensuring that client’s needs are being adequately addressed.

OUR PROCESS

How It Works

  • Preliminary Phone Call – An experienced Care Manager will schedule a preliminary phone call to get basic background information.
  • Comprehensive Assessment – A Care Manager will meet with you privately in your home. She will do a comprehensive assessment; take the time to get to know you, your health background, preferences and concerns. She will review a menu of services that could be helpful for you.
  • Plan of Care – The Care Manager will develop a care plan tailored for each individuals needs. The Plan may be modified in consultation with client and family, as circumstances change.
  • Ready to Go – After the Plan of Care is developed you will decide along with the Care Manager what you would like her to do. Services can be short term to manage a Crisis or long term to manage overall Health Care.

How Can We Help?

It is the policy of McKenney Home Care that each of our trained assistants is compliant with the following:

  • Provides care to the assigned patient(s) under the direction of appropriate clinical supervision.
  • Maintain accurate, complete records regarding patient care including charts, nursing care plans, and evaluation as a regular part of patient care.
  • Cooperate with health care personnel and/or the family to provide continuity of patient care.
  • Maintains a current certificate as appropriate for the level of care.
  • Shows evidence of approved continuing education
  • Ensures confidentiality with regard to the Client/Patient and adheres to the standards established by McKenney Home Care
  • Maintains current CPR and annual TB testing per company policy
  • Adheres to the standards established by the Occupational Safety and Health Administration (O.S.H.A.)

The owners are hands on Managing Members and together with their Director of Nursing offer healthcare and business backgrounds unparalleled in the community.

All McKenney Home Care caregivers are W-2 employees that are bonded and insured, they have access to Healthcare through the “Healthiest You” Program and are specifically trained by the Agency to manage individual cases.

We are accredited by Accreditation Commission for Health Care (ACHC). ACHC is a third-party accrediting organization that has developed the highest national standards that providers are measured against in order to illustrate their ability to effectively and efficiently deliver quality healthcare services.

McKenney Home Care relies on the input of a world renowned Medical Advisory Board, the Members have long term professional relationships with the Medical Advisory Board Members and rely on them for input on sensitive cases and to bring “best practices” to the Agency.

We have a 24/7 on call line which is continuously staffed by one of our staff members. You will not get an answering machine or a call service when you call after hours.
